Business Application

Equipment for Work Place Use

If you work for a small business or agency with no more than fourteen fulltime employees, you may be eligible to apply for a no cost amplified phone, TTY, or other specialized telephone. Please note: To qualify, your business phone number must be different than your home phone number.

Here’s what you need to do. First you must be certified to use this equipment at home. Even if you do not wish to use your equipment at home, you must go through that certification process. The signature of a qualified medical professional establishes your certification.

Once your Certification Form is approved, you will receive a letter or call. CTAP will contact low-vision and blind customers by phone before sending a letter.

The letter or call will instruct you to either get in touch with the State of California CTAP Contact Center or visit a State of California CTAP Service Center.

You will be assessed for the best equipment and services to meet your needs.

Additional training is also available by getting in touch with the State of California CTAP Contact Center or visiting a CTAP Service Center.

Already Qualified for Home Use?

You’ve filled out the Certification Form, had it signed by a medical professional, and returned the form? Great!

All you need to do is to fill out the CTAP Small Business Application.

Ask your supervisor to sign the form. Now send the CTAP Small Business Application to the address below:
